Description
DOL FMLA regulations effective January 16th, 2009, require employers to provide an eligibility notice and a rights and responsibilities notice to an employee requesting leave. These notices must be provided within 5 business days of the leave request. To help covered employers meet this notice requirement, Personnel Concepts has made the DOL's model eligibility notice/ rights and responsibilities notice form available in packs of 25. This form is designed to comply with 29 CFR Sec. 825.300(b). |
